- Send ONE studio check or credit card (payment in full) to the address below by rebate or closing date.

- Print Master Studio WORKSHEET 2006 and Official Entry Form 2006.
- Complete one Master Studio Worksheet and one entry form per routine or per master class style/division.
- Copy each Official Entry Form for your reference.
- Send all entry forms together with signed Master Studio worksheet, ONE studio check or credit card (payment in full) to address below by rebate or closing date.
